FRAILTY, FALLS & MSK CONDITIONS

At the Rainbow Foundation, our support for frailty, falls and musculoskeletal (MSK) conditions is built around a clear, supportive pathway that helps people stay active, independent and confident as they age. We recognise that changes in strength, balance, mobility and pain often overlap and that effective support works best when these needs are addressed together, not in isolation.

 

Our approach brings together the expertise of our Dementia and Frailty Ambassador, alongside our Balanced Futures and Active Futures programmes, offering the right level of help at the right time.

Our Approach

Our Frailty, Falls and MSK support is person‑centred, preventative and strengths‑based. We focus on early identification, confidence‑building and progression supporting people to move from reassurance and gentle activity through to structured, evidence‑based programmes when appropriate.

Frailty Support

Our frailty support is delivered through a joined‑up pathway that combines personalised assessment, gentle movement and structured progression. This ensures people receive support that matches their needs and goals.
We provide:
  • Frailty screening and informal assessments led by our Dementia and Frailty Ambassador
  • Personalised advice, advocacy and reassurance to support voice, choice and control
  • Early‑stage confidence, balance and mobility support through Balanced Futures, our gentle movement and wellbeing sessions
  • Clear progression into Active Futures, our physiotherapist‑led programme supporting strength, mobility, pain reduction and frailty reversal
  • Practical advice to support daily activities, routines and independence
  • Referrals and links to relevant health, social care and community services
This pathway approach supports people to remain resilient, active and independent for as long as possible.

Falls Prevention and Recovery

Falls are often linked to frailty, reduced confidence and MSK conditions. Our falls support focuses on prevention, recovery and rebuilding confidence, using both specialist guidance and movement‑based interventions.
Support includes:
  • Falls risk awareness and personalised prevention advice
  • Balance and strength support through Balanced Futures sessions
  • Progression into Active Futures to improve stability, posture and lower‑limb strength
  • Confidence‑building following a fall, including fear‑of‑falling support
  • Practical home and lifestyle safety guidance
  • Signposting and referral to appropriate specialist services where needed
Our aim is to help people feel safe, steady and confident in everyday movement.

Supporting People With MSK Conditions

Living with MSK conditions such as arthritis, chronic pain or reduced mobility can affect physical comfort, confidence and independence. Our non‑clinical, supportive interventions focus on safe movement, strength and long‑term wellbeing.
We support people through:
  • Pain‑aware movement and activity guidance
  • Gentle re‑introduction to movement through Balanced Futures
  • Structured progression into Active Futures, supporting muscle strength, flexibility, posture and joint stability
  • Confidence‑building around everyday activities and mobility
  • Motivation, encouragement and peer support within group settings
  • Signposting to additional health and wellbeing services
Our approach helps people remain active within their limits while reducing isolation and fear of movement.

Our integrated Frailty, Falls and MSK pathway supports people to build and maintain strength, balance and mobility, reducing the risk of falls and injury while increasing confidence and independence. Through progressive, person‑centred support, individuals are better able to manage pain and long‑term conditions, remain physically active, and continue participating in community life. Strong links to ongoing services and programmes ensure people feel supported, connected and confident in maintaining their wellbeing over time.
