At the Rainbow Foundation, we believe that planning for the future is an important part of wellbeing. Thinking about future care and end of life can feel difficult, but having open, supportive conversations can bring reassurance, clarity and peace of mind – for individuals and for those who care about them.
Why Future Care Planning Matters
Planning ahead allows people to express what matters most to them, make informed choices about their future care and support, and reduce uncertainty and anxiety for themselves and those close to them. It helps ensure that their wishes are understood and respected, even if they are unable to communicate them at a later time. Future care planning is not about giving up hope – it is about living well now, with the reassurance and confidence that your voice will be heard in the future.
Our Approach
Rainbow Foundation supports conversations about future care and end of life in a way that is gentle, respectful and led by the individual. We recognise everyone’s values, beliefs, culture and experiences are different.
Our approach is:
- Person‑led – you decide what you want to talk about and when
- Trauma‑informed and compassionate – sensitive to emotions and past experiences
- Accessible and inclusive – using clear language and supported conversations
- Supportive, not medicalised – focused on wellbeing, dignity and personal choice
What We Can Support With
We offer support to help people think about, record and communicate their wishes, including:
- Conversations about what matters most in life and care
- Understanding future care preferences
- Support with Advance Care Planning discussions
- Thinking about living well with long‑term or life‑limiting conditions
- Considering comfort, dignity, choice and control
- Support for people with dementia and their families to plan ahead
We do not replace legal or clinical services, but we help people feel confident and informed when engaging with them.
Support for Families and Carers
Talking about future care and end of life can be emotionally challenging for families and carers. We provide:
- Safe spaces for open and honest conversations
- Support to understand and respect a loved one’s wishes
- Guidance on communicating choices with professionals
- Emotional support before, during and after planning conversations
Our aim is to reduce fear and conflict, strengthen relationships and promote shared understanding.
A Joined-Up, Compassionate, Community Approach
Future Care & End of Life Planning is closely linked with our wider Rainbow Foundation services, ensuring people and families receive consistent, compassionate support at every stage. This includes Dementia and Frailty Support to help people plan ahead early, Mental Health and Emotional Wellbeing services that recognise the emotional impact of planning, Carer and Family Support to ensure loved ones feel informed and supported, and Community Wellbeing Services that promote dignity, voice and inclusion.
We believe conversations about future care and end of life belong in communities, not just clinical settings. By taking a community‑based approach and working across services, we help people feel less alone, more empowered and better understood, while building trust and supporting individuals to maintain dignity, choice and control over what matters most to them.
